Our eldest son, JJ, who was 13 years old at that time, traveled to the USA all by himself.

Before a minor can travel abroad unaccompanied by any parent or legal guardian, a DSWD permit is needed.

Here are the requirements and procedure for obtaining a DSWD permit for unaccompanied minor travelling overseas:

I PROCEDURE:

  1. Fill out the DSWD Application Form. Click on this link to download the form.
  2. Submit the accomplished form together with the following documents:

          i. Photocopy of Birth Certificate or Passport of the minor;
          ii. Written consent of both parents or Legal Guardian. In JJ’s case, however, I was the only one who accomplished the written consent because my wife is in the US.
          iii. Photocopy of Marriage Certificate of minor’s parents or Certificate of Legal Guardianship if the minor is under guardianship.
          iv. If the minor is visiting a relative or friend abroad, a written acknowledgement of support by the relative or friend MIGHT be required like in our case. The document should bear the original signature of the relative or friend. In the absence of the acknowledgement letter, you will be required to execute a waiver duly notarized by a notary public.
           v. 2 colored passport size photos of the minor taken within the past 6 months.

NOTE: Although Photocopy is stated, it is prudent to bring with you the original document just in case they need to see it.

He will be staying with his grandmother, Juanita dela Cruz-Smith, resident of 49 Viewcrest Cescent, Chicago, IL USA, with Passport No. ABC012345678.

I hereby release the DSWD and other Philippine government agency from any liability whatsoever arising from the travel of my child.

NOTE2: If the minor is traveling with a person other than the parent/s or legal guardian, a photocopy of the passport of the traveling companion should be presented.

II. FEES:

Php 300.00 with validity of one (1) year.
Php 600.00 with validity of two (2) years.

III. How long does it take for the certificate to be released?

It took 5 working days for the permit to be released from the day I submitted the application. The ideal time frame should be at least 2 months prior to the date of departure of the minor to give some leeway for unexpected delays specially for those with extraordinary circumstance like common names, missing documents etc.
